Use a file-upload question when you want people to provide supporting documents or images as part of a request.

Where to find it

Open the request page in the Editor, open Questionnaire > Questions, then add or edit a question.

Choose file upload

Select the File upload question type and write a clear prompt explaining what should be provided.

Files submitted through the questionnaire are attached to the resulting request so your team can review them with the other information collected.
