Set the headline and supporting text people see before answering questionnaire questions.

The questionnaire introduction explains what information you are asking for and what happens next.

Where to find it

Open the request page in the Editor, open Questionnaire, then select Introduction.

Introduction fields

  • Headline — the first line shown before the questions.
  • Additional text — a short explanation of why the information is being collected.
  • If a submission needs review — the message shown when a submission is held for review instead of proceeding directly to scheduling.

Save your changes, preview the page and publish when you are ready for the live page to use them.
